Kidney stones are a common condition with an annual incidence of eight cases per 1,000 adults. Renal colic must first be ruled out for conditions that require immediate admission to the emergency department, and then the pain must be relieved, preferably with a nonsteroidal anti-inflammatory drug. Diagnostic testing consists of urinalysis, urine culture, and imaging to confirm the diagnosis and to determine if active stone removal is required, such as for a urinary tract infection or a stone greater than 10 mm in size. Conservative management consists of pain control, drug elimination therapy with an alpha blocker, and follow-up within 14 days to monitor the location of the stone and detect hydronephrosis. Asymptomatic kidney stones should be monitored with regular imaging and removed if there is growth, symptoms, urinary tract obstruction, recurrent infection, or lack of access to medical care. All patients with kidney stones should be evaluated for risk of stone recurrence based on history, basic laboratory tests, and imaging. All patients should be advised of lifestyle changes such as increased fluid intake, and patients with recurrent calcium stones should be prescribed thiazide diuretics, allopurinol, or citrate. Patients at high risk for recurrent stones should be referred for additional metabolic testing, which can serve as the basis for tailored preventive measures.
